As rx10 said, there is a gap between your concept knowledge and application. I would suggest you to go thorough the GRE club maths book it would be of much help. As for verbal, try to improve more on rc by reading diverse articles and using the strategies answer a particular question

Hey Ayush, You can try attempting questions from this forum. Apply filter and practice. Just avoid silly mistakes.

Try making short summaries of each para and then link them. You will get the whole idea of the passage. It will take time for sure but improvement will be there.
