If his hands were trembling then his portrait must have been anything but perfect. Hence, inexact is the answer.

It required no foresight to anticipate that Rafnkelsson’s inaugural self-portrait would be a(n) ______________ one, given that his long-standing illness causes his hands to tremble.

The portrait would be something because he had a long standing illness which caused his hands to tremble.

Clearly the portrait would not be perfect, due to his trembling hands, and hence inexact is a good choice. Also, there are no clues to suggest that the end result was macabre, so we reject that choice (even though an inexact portrait could have a macabre effect).
