The key word is internal. You need to pair with a word in the opposite meaning: tropical (external) is the answer.

Answer must be topical, not tropical!

Topical means "pertaining to the surface of a body part"

It should be topical indeed! However this is a highly medical term. I doubt it gets asked in GRE!
