This question is pretty easy if we take a number as an example. For sake of simplicity, let's take 100. Then, 25 percent of 100 is equal to 25, while a 25 percent increase applied to 100 gives 125. Now, we have to check which operations on 25 gives us 125.

Adding x means adding 100 to 25, which gives us 125 -> CORRECT
Multiplying by x means to compute 25*100 = 250 -> WRONG
Dividing by 0.25 means to compute 25/0.25 = 100 -> WRONG
Multiplying by 5 means computing 25*5 = 125 -> CORRECT

The answers are A and D!
